Banner Access Request form (BAR form)
1. This form must be completed by the employee's supervisor. (Employees may not request their own Banner access)
2. Once the form is completed, the form emails the Help Desk email account (CSC@STLCC.EDU).
3. The contents of the email is copied into a ticket using the BAR Form template.
Note: The requesting manager should be linked to the requestor field
Note: The short description of the ticket should be "Bar Form - username of employee needing access"
4. All Banner teams should automatically be added to the ticket once the BAR Form template is assigned to the ticket.
